Integrating Acute to Post Acute Care Settings: Where Do We Go From Here?

This event has been approved for 1.5 Virtual Interactive Education Hours by ACHE.  This Virtual Interactive Event will be held in the ACHE Learning Management System. Registration for this event will close TWO DAYS prior to the actual event.  Use of the ACHE Learning Management System will allow the attendees’ education credits to automatically be applied to their MY ACHE account, based upon participation in the event.  After you check out on the ACHE website, you will receive further instructions to the email address which you provided at checkout with directions to complete your registration for this event.

Management of populations of patients through integrated post-acute care models cannot be done by a simple referral.  As our populations changes, so must the delivery of healthcare services along the care continuum.  Acute care hospitals are faced with ever more regulatory requirements and expanding financial penalties all to elevate the standard of care across the healthcare system on a national scale. The regulatory requirements and the corresponding financial penalties are not new phenomena.

Most hospitals aggressively develop performance improvement plans that center on the resolution of these deficits for minimization of potential financial exposure. Driving forces are pushing acute care hospitals to work collaboratively with members of the post-acute care continuum to cohesively manage patient populations through increasingly more aspects of a patient’s post-acute care progression. As a result, these post-acute facilities will continue to play an even greater role in supporting our acute care hospitals in the management of patient populations through the continuum of care.
